This monograph on the "Ultrastructure of Human Sella Tumors" is in fact a study of the correlations of clinical findings and morphology. It is a timely and eagerly awaited publication because of the increasing interest of the endocrinologist in pituitary disorders and of the neuro surgeon in the newest aspects of surgery on pituitary tumors, and also because of the unsatisfactory but still widely accepted classification into eosinophil, basophil and chromophobe pituitary adenomas. This old classification has been mainly based on granule color as seen after hematoxylin-eosin staining, but it does not, after extensive clinical observations, reflect in many instances the type of clinical picture observed. The author has brilliantly succeeded in demonstrating in a convinc ing way, by an extensive study of the relevant literature and by his own histological work, that further insight into the biology of the hypophysis and of the pituitary tumor can be obtained only by newer methods of light microscopic staining, immuno-histochemistry and electron microscopy combined with radio-immunological blood hormone determination.

Since the time of the earliest electron microscopic studies on tumours of the human nervous system. undertaken over 20 years ago by Luse and her colleagues. there have been considerable advances in our understanding of these neoplasms. Tissue culture and specific antibodies to tumour antigens are two of the techniques which have greatly aided such advances. enabling much to be learned about the biological properties and underly ing nature of all types of nervous system tumour. Electron microscopy. however. has continued to prove of considerable value in the investigation of these tumours. and the technological advances of the last two decades have dramatically improved the resolution and overall quality of the ultrastructural images obtained. In clinical neuropathology. such improvements have encouraged a more widespread use of the electron microscope in the diagnosis of human nervous system tumours. alongside antibody techniques and more conventional histological methods. Although ultrastructural examination is often not strictly necessary to identify well-differentiated tumours such as astrocytomas or choroid plexus papillomas. study of the electron microscopic features in such cases fre quently proves useful in the diagnosis of their more malignant counterparts. Thus the recognition of astrocytic filaments in anaplastic gliomas. or of cilia in pleomorphic choroid plexus carcinomas. may enable a diagnosis to be made in cases where there is insufficient differentiation for the tumours to be recognised at light microscopic level.

The American Cancer Society anticipates that 16,500 patients will be diagnosed with primary malignant tumors of the central nervous system in 2000, with about 200,000 individuals presenting with brain metastases. The advances in the treatment of solid tumors have contributed significantly to the major increase in metastatic cancers to the brain. Of the primary malignant tumors of the brain, more than 50% are high-grade gliomas; the incidence has been increasing among older patients over the past decade. Major developments in new technologies in the treatment of primary brain tumors as well as metastatic disease are covered in depth. Even though management is difficult, advances are being made. This book is a concerted effort to present data regarding basic science research efforts alongside their translation into clinical practice using combined, integrated multimodal programs of treatment. Progress has been made, but innovatice approaches need to be pursued.
