As a relatively young discipline, health economics as it appears today contains many features which can be traced back to its beginnings. Since it arose in the interface between the medical sciences and economics, the way of dealing with problems were often influenced by traditions which were well-established in the medical profession, while the classical way of thinking of economists came was filtering through at a slower pace. This means that much of both teaching and research in health economics puts the emphasis on collecting and analysing data on health and healthcare as well as on public and private outlays on healthcare. This is an extreme useful and worthwhile activity, and much new and valuable information is produced in this way, but occasionally there is a need for in-depth understanding of what is going on, rather than an estimated equation which comes from nowhere. This is where economic theory can offer some support.The present book is an introduction to health economics where the emphasis is on theory, with the aim of providing explanation of phenomena as far as possible given the current level of economics.

This book provides a robust set of health economic principles and methods to inform societal decisions in relation to research, reimbursement and regulation (pricing and monitoring of performance in practice). We provide a theoretical and practical framework that navigates to avoid common biases and suboptimal outcomes observed in recent and current practice of health economic analysis, as opposed to claiming to be comprehensive in covering all methods. Our aim is to facilitate efficient health system decision making processes in research, reimbursement and regulation, which promote constrained optimisation of community outcomes from a societal perspective given resource constraints, available technology and processes of technology assessment. Importantly, this includes identifying an efficient process to maximize the potential that arises from research and pricing in relation to existing technology under uncertainty, given current evidence and associated opportunity costs of investment. Principles and methods are identified and illustrated across health promotion, prevention and palliative care settings as well as treatment settings. Health policy implications are also highlighted.

Most economic evaluations of health care programmes at the moment are cost effectiveness and cost-utility analyses. The problem with these methods is that their theoretical foundations are unclear. This has led to confusion about how to define the costs and health effects and how to interpret the results of these studies. In the environmental and traffic safety fields it is instead common to carry out traditional cost-bene:fit analyses of health improving programmes. This striking difference in how health programmes are assessed in different fields was the original motivation for writing this book. The aim of the book is to tty and provide a coherent framework within cost-bene:fit analysis and welfare economics for the different methods of economic evaluation in the health care field. The book is written in an easily accessible manner and several examples of applications of the different methods are provided. It is my hope that it will be useful both for teaching purposes and as a guide for practitioners in the field. Glenn C. Blomquist, John D.
