A wave of new health care innovation and growing demand for health care, coupled with uncertain productivity improvements, could severely challenge efforts to control future health care costs. A committee of the National Research Council and the Institute of Medicine organized a conference to examine key health care trends and their impact on medical innovation. The conference addressed the following question: In an environment of renewed concern about rising health care costs, where can public policy stimulate or remove disincentives to the development, adoption and diffusion of high-value innovation in diagnostics, therapeutics, and devices?

This study has emerged from an ongoing program of trilateral cooperation between WHO, WTO and WIPO. It responds to an increasing demand, particularly in developing countries, for strengthened capacity for informed policy-making in areas of intersection between health, trade and IP, focusing on access to and innovation of medicines and other medical technologies.

Technological development has created major possibilities for the treatment of disease and for the disabled. The cost of new technologies has added considerably to health care cost intlation, which still exceeds the growth rates of most national economies. The share of national resources devoted to health care is still rising, although at a lesser pace than in the seventies. -Therefore, the use of medical technology confronts us with some of the major dilemmas in society today. The routine and intensive use of technology has transformed the most basic interpersonal and social features of medicine. It has altered the means through which patient and doctor communicate about illness as well as the content of this communication, changed the doctor's relationship to medical colleagues by increasing his dependence on them, altered the place and form of practice by creating advantages for the centralization of medical care in complex organizations, and created for society new responsibilities and powers to influence the context and scope of medical practice.

This book provides a robust set of health economic principles and methods to inform societal decisions in relation to research, reimbursement and regulation (pricing and monitoring of performance in practice). We provide a theoretical and practical framework that navigates to avoid common biases and suboptimal outcomes observed in recent and current practice of health economic analysis, as opposed to claiming to be comprehensive in covering all methods. Our aim is to facilitate efficient health system decision making processes in research, reimbursement and regulation, which promote constrained optimisation of community outcomes from a societal perspective given resource constraints, available technology and processes of technology assessment. Importantly, this includes identifying an efficient process to maximize the potential that arises from research and pricing in relation to existing technology under uncertainty, given current evidence and associated opportunity costs of investment. Principles and methods are identified and illustrated across health promotion, prevention and palliative care settings as well as treatment settings. Health policy implications are also highlighted.
