Growing evidence suggests that in order to improve a population's health in a significant and sustained way there must be clear direction, commitment, and effective collaboration between the many entities that provide care or engage in community improvement. Effective collaborations can make positive strides in addressing the social determinants of health, which affect population health. Hospitals play a significant role in care delivery, from basic attention to the most sophisticated interventions, but certainly all organizations want to provide the highest quality services to the populations they serve. State health departments can play an important role in driving what hospitals are doing, convening this collaboration, and facilitating success at the local level. This perspective discusses challenges and barriers that state and local partners face related to collaborating on the above needs assessments, as well as examples of successful state strategies that can overcome these barriers.

On February 5, 2015, the Institute of Medicine Roundtable on Population Health Improvement hosted a workshop to explore the relationship between public health and health care, including opportunities, challenges, and practical lessons. The workshop was convened in partnership with the Association of State and Territorial Health Officials (ASTHO)-Supported Primary Care and Public Health Collaborative. Organized in response to the 2012 IOM report Primary Care and Public Health: Exploring Integration to Improve Population Health, this workshop focused on current issues at the interface of public health and health care, including opportunities presented by and lessons learned from the Centers for Medicaid and Medicare Services State Innovation Models program. The workshop featured presentations on several dimensions of the public health-health care relationship. Collaboration Between Health Care and Public Health summarizes the presentations and discussion of the event.

In the United States, some populations suffer from far greater disparities in health than others. Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States.
