For many years, there has been a great deal of work done on chronic congestive heart failure while acute heart failure has been considered a difficult to handle and hopeless syndrome. However, in recent years acute heart failure has become a growing area of study and this is the first book to cover extensively the diagnosis and management of this complex condition. The book reflects the considerable amounts of new data reported and many new concepts which have been proposed in the last 3-4 years looking at the epidemiology, diagnostic and treatment of acute heart failure.

Nearly half of the world's eight million Palestinians are registered refugees, having faced partition and exile. Landscape of Hope and Despair examines this refugee experience in Lebanon through the medium of spatial practices and identity, set against the backdrop of prolonged violence. Julie Peteet explores how Palestinians have dealt with their experience as refugees by focusing attention on how a distinctive Palestinian identity has emerged from and been informed by fifty years of refugee history. Concentrating ethnographic scrutiny on a site-specific experience allows the author to shed light on the mutually constitutive character of place and cultural identification. Palestinian refugee camps are contradictory places: sites of grim despair but also of hope and creativity. Within these cramped spaces, refugees have crafted new worlds of meaning and visions of the possible in politics. In the process, their historical predicament was a point of departure for social action and thus became radically transformed. Beginning with the calamity of 1948, Landscape of Hope and Despair traces the dialectic of place and cultural identification through the initial despair of the 1950s and early 1960s to the tumultuous days of the resistance and the violence of the Lebanese civil war and its aftermath. Most significantly, this study invokes space, place, and identity to construct an alternative to the received national narratives of Palestinian society and history. The moving stories told here form a larger picture of these refugees as a people struggling to recreate their sense of place and identity and add meaning to their surroundings through the use of culture and memory.

The wave of anti-government protests that swept through the Arab world from December 2010 on started to transform politics and society in the Middle East. The protests came as a surprise to many observers-- but not to Rami Zurayk, an veteran Lebanese agronomist and social activist who had been analyzing the collapse of traditional agricultural livelihoods in the Middle East since the late 1980s. In 2007, Zurayk started writing the "Land and People" blog, which charts food-policy and agricultural policy issues throughout the Middle East. Food, Farming, and Freedom presents his choice of the best of the posts in the blog from 2007 through April 2011. It concludes with a chapter tracking the early months of the Arab Spring.

Associating social justice with landscape is not new, yet the twenty-first century's heightened threats to landscape and their impact on both human and, more generally, nature's habitats necessitate novel intellectual tools to address such challenges. This book offers that innovative critical thinking framework. The establishment of the Universal Declaration of Human Rights (UDHR) in 1948, in the aftermath of Second World War atrocities, was an aspiration to guarantee both concrete necessities for survival and the spiritual/emotional/psychological needs that are quintessential to the human experience. While landscape is place, nature and culture specific, the idea transcends nation-state boundaries and as such can be understood as a universal theoretical concept similar to the way in which human rights are perceived. The first step towards the intellectual interface between landscape and human rights is a dynamic and layered understanding of landscape. Accordingly, the 'Right to Landscape' is conceived as the place where the expansive definition of landscape, with its tangible and intangible dimensions, overlaps with the rights that support both life and human dignity, as defined by the UDHR. By expanding on the concept of human rights in the context of landscape this book presents a new model for addressing human rights - alternative scenarios for constructing conflict-reduced approaches to landscape-use and human welfare are generated. This book introduces a rich new discourse on landscape and human rights, serving as a platform to inspire a diversity of ideas and conceptual interpretations. The case studies discussed are wide in their geographical distribution and interdisciplinary in the theoretical situation of their authors, breaking fresh ground for an emerging critical dialogue on the convergence of landscape and human rights.

Health systems should function in such a way that the amount of inappropriate care is minimized, while at the same time stinting as little as possible on appropriate and necessary care. The ability to determine and identify which care is overused and which is underused is essential to this functioning. To this end, the "RAND/UCLA Appropriateness Method" was developed in the 1980s. It has been further developed and refined in North America and, increasingly, in Europe. The rationale behind the method is that randomized clinical trials--the "gold standard" for evidence-based medicine--are generally either not available or cannot provide evidence at a level of detail sufficient to apply to the wide range of patients seen in everyday clinical practice. Although robust scientific evidence about the benefits of many procedures is lacking, physicians must nonetheless make decisions every day about when to use them. Consequently, a method was developed that combined the best available scientific evidence with the collective judgment of experts to yield a statement regarding the appropriateness of performing a procedure at the level of patient-specific symptoms, medical history, and test results. This manual presents step-by-step guidelines for conceptualising, designing, and carrying out a study of the appropriateness of medical or surgical procedures (for either diagnosis or treatment) using the RAND/UCLA Appropriateness Method. The manual distills the experience of many researchers in North America and Europe and presents current (as of the year 2000) thinking on the subject. Although the manual is self-contained and complete, the authors do not recommend that those unfamiliar with the RAND/UCLA Appropriateness Method independently conduct an appropriateness study; instead, they suggest "seeing one" before "doing one."
