The International Reporting System for Operating Experience (IRS) is an essential element of the international operating experience feedback system for nuclear power plants. Its fundamental objective is to contribute to improving safety of commercial nuclear power plants which are operated worldwide. IRS reports contain information on events of safety significance with important lessons learned which assist in reducing recurrence of events at other plants. This sixth publication, covering the period 2012 - 2014, follows the structure of the previous editions. It highlights important lessons based on a review of the approximately 240 event reports received from the participating countries over this period.

A guidebook to the power reactors of the world, operating or under construction.

Reliability data collection and its use in risk and availability assessment is a subject of increasing importance. The founders of EuReDatA, and in particular, Arne Ullman, the originator 'and first Chairman of the Association, recognised the need for a body capable of acting as a catalyst and providing a unified approach to this subject. It is therefore a prevailing objective of the European Reliability Databank Association to initiate and support contact between experts, companies and institutions active in reliability engineering and research. Although the first and principle interest of EuReDatA is reliability data and data banks, the Association is aware that these are tools that are used with others to establish and maintain reliability and safety. On the basis of the principles included in the Fundamental Safety Principles, IAEA Safety Standards Series No. SF-1, this Safety Requirements publication establishes requirements applicable to the design of nuclear power plants. It covers the design phase and provides input for the safe operation of the power plant. It elaborates on the safety objective, safety principles and concepts that provide the basis for deriving the safety requirements that must be met for the design of a nuclear power plant. Member States intending to introduce a nuclear power programme will need to pass through several phases during the implementation. Experience shows that careful planning of the objectives, roles, responsibilities, interfaces and tasks to be carried out in different phases of a nuclear project is important for success. This publication presents a harmonized approach that may be used to structure the owner/operator management system and establish and manage nuclear projects and their development activities irrespective of the adopted approach. It has been developed from shared management practices and consolidated experiences provided by nuclear project management specialists through a series of workshops and working groups organized by the IAEA. The resultant publication presents a useful framework for the management of nuclear projects from initiation to closeout and captures international best practices.

Effectively utilising and disseminating operating experience is one of the major elements of an operating experience feedback programme. Although not recognised prior to the event, most subsequent investigations identify internal and external industry operating experience that, if applied effectively, would have prevented the event. The use of the guidance provided by this publication will be helpful to nuclear installations, and related institutions including contractors and support organisations to strengthen and enhance their own feedback process through the implementation of best practices in the utilisation and dissemination process and to assess the effectiveness of the above area. This publication develops some of the principles set forth in the PROSPER guidelines issued by the IAEA in 2003 to provide advice and assistance to nuclear installations to strengthen and enhance their own operating experience programmes.
