A critical appraisal of recent achievements What at the beginning of the last century started as a struggle to develop new devices for the endoluminal exploration of the digestive system has resulted in unexpected achievements in the field of advanced high-tech endoscopy such as interventional endoscopy and refinements in diagnostic resolution. This publication reviews some of the established major progress which has recently been made with regard to diagnostic and interventional endoscopy, including methods that are currently becoming part of today's clinical management. Endoscopy is a fast moving field, and new techniques are continuously emerging. In recent decades, endoscopy has evolved and branched out from a diagnostic modality to enhanced video and computer assisting imaging with impressive interventional capabilities. The modern endoscopy has seen advances not only in types of endoscopes available, but also in types of interventions amenable to the endoscopic approach. Endoscopy is an effective and safe procedure even in special populations including pediatric patients and renal transplant patients. It serves as the tool for diagnosis and therapeutic interventions of many organs including gastrointestinal tract, head and neck, urinary tract and others.

Endoscopic techniques are widely used for screening, diagnostic and therapeutic maneuvers in all groups of patients and for a large spectrum of complaints. The availability of basic iterations of endoscopic techniques made screening programs for various diseases viable in most parts of the world, while the advent of modern techniques opens new perspectives for rapid and correct diagnosis. Going beyond normal human vision, innovative techniques opened the prospect of in-situ pathology. Endoscopic ultrasound has made incredible progress in recent years. Reaching the smaller orifices by endoscopy was a major step forward in the surveillance of previously inaccessible lesions. Investigatory techniques were complemented by advances in therapy, with novel applications in many major areas of medicine.
