Created out of existing Marine forces and resources, the Chemical Biological Incident Response Force's responsibilities include reconnaissance, detection, decontamination, medical, security, and service support for managing crisis situations at home and abroad. It is a special Marine unit of first responders designed to be a well-trained, quick-moving group of soldiers, doctors, and scientists who can be mobilized to detect, prevent, or respond to terrorist threats or catastrophes that involve biological weapons, chemical weapons, or nuclear agents. In our current chaotic, destabilized, and threatening global environment, CBIRF may be the only thing that stands between us and a horrific and deadly new form of warfare waged on innocent civilian populations.

The Marine Corps Chemical Biological Incident Response Force (CBIRF) was established in 1996. Since then, CBIRF s mission has expanded to included responses to radiological and high-yield explosive incidents. Its concept of operation has been reworked to include two separate incident response forces within CBIRF. The objective of this study was to determine the medical requirements for a variety of potential terrorism scenarios to which CBIRF may respond. These included domestic and international attacks involving high-yield explosives as well as chemical, biological, and radiological agents. This study was able to identify weaknesses in the existing CBIRF Authorized Medical Allowance Lists, and to strengthen the field medical capabilities of the unit with the addition of new technologies, such as portable ultrasound units to aid in the examination of severely injured victims. At the same time, the study identified ways of modularizing the proposed CBIRF AMAL to provide greater flexibility in responding to terrorist disasters.

First responders: fire, rescue, law enforcement, and emergency medical services provide immediate aid and support for safeguarding public health and safety. Second Tier - State Response. For example, additional civilian teams, such as State Hazardous Materials (HAZMAT) teams, Fire and Rescue units, State Police units, State Health Department assets, and the NG are used to augment first responder resources. Third Tier - Federal Response. Governors can request assistance from the President of the United States if it is determined their State's incident-response requires additional support. An incident command (IC) is established to provide assistance and management of the emergency support functions of the incident. Federal response assets available to the IC may include the Federal Bureau of Investigation (FBI) HAZMAT Response Unit, the Department of Energy's radiation assessment teams, Federal Emergency Management Agency (FEMA), the National Disaster Medical System, the National Medical Response System, the NG, United States Northern Command (USNORTHCOM) and United States Pacific Command (USPACOM).

The United States has established a national policy for combating chemical and biological terrorism and managing the consequences of terrorist attacks. In the event of a domestic chemical or biological terrorist incident, local and state governments would be the first to respond in assisting civilian victims. If the consequences of such an incident overwhelmed state and local capabilities, federal assistance could be given to support their efforts. Critical to that assistance are the chemical and biological medical supplies maintained by OEP, CDC, VA, and CBIRF.
