Back pain is a common musculoskeletal condition that presents to physicians in the primary care office, the urgent care facility, and the emergency room. However, few primary care physicians have received education and training about the appropriate workup and referral for a child who presents with back pain. This book is designed to fill the gaps in primary care physician knowledge and ultimately improve patient care. This book is divided into three major sections. The first section contains seven introductory chapters on epidemiology, anatomy of the spine, imaging studies, clinical considerations, general history questions, and physical examination pearls of the child with back pain. This section gives an overview of common back pain problems and provides instructions on how to best perform a history and examination of the pediatric patient with back pain. Classic historical and examination findings are highlighted that can lead to a focused differential diagnosis, successful treatment and appropriate referral. Recognition of the red flags in the history and physical keep the clinician ‘out of trouble‘. This section concludes with a chapter that guides physicians in "putting it all together". The second section is the largest section of the book and contains 25 case based chapters organized by major presenting clinical features. Cases vary from common presentations of back pain such as back pain related to back packs to rare presentations of back pain such as back pain stemming from tuberculosis. A foreword might be a preview of things to come, but a “back word” of what was thought to be the ?nal say on the subject is needed for a perspective in progress. A “back word” look reveals the tremendous progress in medical diagnosis and treatment of which paediatric orthopaedics and fracture care is a component. Clubfoot treatment based on the dictums of Hiram Kite has had a revolutionary change by Ponseti. The chapter by Eastwood has the details on cast application and orthotics follow-up to obtain the 95% correction without the extensive surgery many of us thought was needed.

Pain is an increasingly common symptom in children and adolescents. Once recurrent pain leads to pain-related disability that affects a child’s functional, emotional and social well-being, it is considered a chronic pain disorder. Internet use (e.g., smartphone use, social media use) is ubiquitous in our daily lives and plays an increasingly important role in human well-being. Today’s adolescents and their parents spend significant amounts of time on technological devices, such as smartphones, especially during COVID-19. In this year, new research shows that nearly half of American teenagers say they use the Internet “almost constantly”, 95% of American teens have smartphones, and about 54% of teens said it would be “somewhat hard” to give up social media. Hence, further research on the relationship between Internet use and psychological well-being among children and adolescents is worth pursuing. At the same time, a new interest in this area of research is “parental technoference” which was defined as regular interruptions to real-time face-to-face communications, interactions, or time spent together among family members because of parental use of technology. Technoference has become a common phenomenon in many families with children, and evidence has been identified that extended parental time on technological devices (e.g., smartphones) could have negative effects on parent-child relationships and children’s wellbeing. For example, parental phubbing is regarded as a typical parental technoference, and this term is used widely in some countries around the world, which has been demonstrated to be detrimental to their children’s developmental outcomes and cause various psychological health problems.
