A spirituality discourse in substance abuse treatment offers useful unconventional constructs in treatment services to ethnic minority groups with substance use disorder (SUDs). It is important to locate spirituality within culture, place, and history in order to understand the spiritual needs of persons from minority groups with SUDs. There are many studies that merit a spiritual approach in treatment for ethnic minority groups with SUDs. However, spirituality is a broad concept that means different things to different people. Therefore, such an unconventional approach should be approached critically and cautiously. This chapter looks at the utilisation of an integrated eclectic approach with a focus on inclusion of spirituality in treatment services from a biopsychosocial-spiritual perspective. Tapping into the spiritual needs and the meaning that people ascribe to spirituality and religion (S&R) in treatment services is often more valued than conventional secular treatment services. Also, the client,Äôs spirituality is generally overlooked by professionals offering such services simply because it is so controversial. This chapter proposes an integrated eclectic methodology calling for a biopsychosocial-spiritual perspective to address the needs and well-being of ethnic minority groups with SUDs as a comprehensive person-centred and holistic approach, utilising mindfulness techniques.

Despite the significant burden of substance use disorders (SUD) across the world, SUD treatment systems face significant challenges to ensure immediate access to effective care. Prevention and treatment of SUDs is generally underfunded and its effectiveness to reduce substance use is frequently questioned. This book presents system level factors that may contribute to improving SUD prevention and treatment, particularly for underserved, and racial and ethnic minority populations. The authors provide theory-based approaches to prevention, as well as evidence-based spiritual, managerial, and leadership approaches to improve the effectiveness of SUD treatments. This book presents an in-depth qualitative study carried out with inpatients under treatment for substance use disorders (SUDs) in seven therapeutic communities (TCs) located in three countries: Peru, Nicaragua and Czech Republic. By comparing the experiences in these different cultural contexts, the book presents a grounded theory of SUDs treatment in TCs from a cross-cultural perspective, identifying factors that influence the efficacy of SUDs treatment in TCs based on interviews carried out with inpatients. Based on rigorous qualitative research methods, this book presents not only a comparative analysis of TCs located in different cultural contexts, but also analyzes the cross-cultural nature of the therapeutic programs adopted in these communities, such as the combination of traditional Amazonian medicine based on the therapeutic use of ayahuasca with conventional psychotherapy and occupational therapy, among other approaches. Departing from the interviews carried out with inpatients, the authors present a comparative analysis of how the different TCs address important issues related to SUDs treatment, and complement this analysis with machine-generated summaries of relevant scientific papers. These summaries contain results of similar research projects conducted in other cultural contexts. Substance Use Disorders Treatment in Therapeutic Communities: A Cross-Cultural Approach presents the results of a unique comparative study with great translational potential which will be of interest to both researchers and practitioners working in TCs. This unique comparative study identifies factors affecting the efficacy of therapeutic programs and proposes a grounded theory which aims to serve as an important source of information for therapists and other professionals working with SUDs treatment and for the replication of applied therapeutic methods in other TCs.

Addiction Treatment is an ethnography that compares two types of residential drug-free treatment programs-religious, faith-based programs and science-based, secular programs. Although these programs have originated from significantly different ideological bases, in examining the day-to-day operations of each, Daniel E. Hood concludes that they are far more alike than they are different. Drug-free treatment today, whether in secular or religious form, is little more than a remnant of the temperance movement. It is a warning to stop using drugs. At its best, treatment provides practical advice and support for complete abstinence. At its worst, it demeans users for a form of behavior that is not well understood and threatens death if they do not stop. Hood argues that there is no universal agreement on what addiction is and that drug abuse is little more than a catch-all term of no specific meaning used to condemn behavior that is socially unacceptable. Through extensive participatory observations, intimate life history interviews, and informal conversations with residents and staff, Hood shows how both programs use the same basic techniques of ideological persuasion (mutual witnessing), methods of social control (discourse deprivation), and the same proposed zero tolerance, abstinent lifestyle (Christian living vs. Right living) as they endeavor to transform clients from addicts to citizens or from sinners to disciples.

Motivation is key to substance use behavior change. Counselors can support clients' movement toward positive changes in their substance use by identifying and enhancing motivation that already exists. Motivational approaches are based on the principles of person-centered counseling. Counselors' use of empathy, not authority and power, is key to enhancing clients' motivation to change. Clients are experts in their own recovery from SUDs. Counselors should engage them in collaborative partnerships. Ambivalence about change is normal. Resistance to change is an expression of ambivalence about change, not a client trait or characteristic. Confrontational approaches increase client resistance and discord in the counseling relationship. Motivational approaches explore ambivalence in a nonjudgmental and compassionate way.
