This book presents a comprehensive neuropsychodynamic strategy for treating psychiatric disorders. Rather than pursuing an exclusively biological, psychological, or psychodynamic approach, it offers a methodology that links all three aspects in a unifying, integrative model. Central to this approach is the view of the brain as a bio-psychosocial organ in a neuro-ecological model, rather than the purely neuronal model often presupposed in current neuroscience and psychiatry. Moreover, the book views psychopathological symptoms as spatiotemporal disorders of the altered spatiotemporal structure spanning the brain and its surrounding world. The relation between one of the core symptoms and altered neuronal activity calls for the development of integrated, circular neuropsychodynamic models of psychopathological symptoms in severe psychiatric disorders and their treatment.

As a discipline, psychoanalysis began at the interface of mind and brain and has always been about those most basic questions of biology and psychology: loving, hating, what brings us together as lovers, parents, and friends and what pulls us apart in conflict and hatred. These are the enduring mysteries of life and especially of early development-how young children learn the language of the social world with its intertwined biological, genetic, and experiential roots and how infants translate thousands of intimate moments with their parents into a genuine, intuitive, emotional connection to other persons. Basic developmental neuroscience and psychology has also of late turned to these basic questions of affiliation: of how it is that as humans our most basic concerns are about finding, establishing, preserving, and mourning our relationships. These areas in broad strokes are the substance of mind and brain, and the last decade has brought much new science to the biology of attachment, love, and aggression.

Adrian Johnston and Catherine Malabou defy theoretical humanities' deeply-entrenched resistance to engagements with the life sciences. Rather than treat biology and its branches as hopelessly reductive and politically suspect, they view recent advances in neurobiology and its adjacent scientific fields as providing crucial catalysts to a radical rethinking of subjectivity. Merging three distinct disciplines—European philosophy from Descartes to the present, Freudian-Lacanian psychoanalysis, and affective neuroscience—Johnston and Malabou triangulate the emotional life of affective subjects as conceptualized in philosophy and psychoanalysis with neuroscience. Their experiments yield different outcomes. Johnston finds psychoanalysis and neurobiology have the potential to enrich each other, though affective neuroscience demands a reconsideration of whether affects can be unconscious. Investigating this vexed issue has profound implications for theoretical and practical analysis, as well as philosophical understandings of the emotions. Malabou believes scientific explorations of the brain seriously problematize established notions of affective subjectivity in Continental philosophy and Freudian-Lacanian analysis. She confronts philosophy and psychoanalysis with something neither field has seriously considered: the concept of wonder and the cold, disturbing visage of those who have been affected by disease or injury, such that they are no longer affected emotionally. At stake in this exchange are some of philosophy's most important claims concerning the relationship between the subjective mind and the objective body, the structures and dynamics of the unconscious dimensions of mental life, the role emotion plays in making us human, and the functional differences between philosophy and science.

The chapters in the second part of the book on Developmental Affective Neuroscience and Developmental Neuropsychiatry address the science that underlies regulation theory’s clinical models of development and psychopathogenesis. Although most mental health practitioners are actively involved in child, adolescent, and adult psychotherapeutic treatment, a major theme of the latter chapters is that the field now needs to more seriously attend to the problem of early intervention and prevention.

A bold look at the body as a source of contention for those who suffer from personality disorders. This work connects interpersonal neurobiology, attachment theory, and psychoanalytic theory with cognitive and neuroscientific work on implicit memory, trauma theory, and dissociation to propose an integrated method for treating severe borderline and narcissistic disorders, with the prime aim of resolving the affect dysregulation that affects the various realms of bodily discomfort and existential pain. Each chapter presents a particular case and illustrates the methods for working with the specific problems that arise: from bulimia to self-cutting to sexual identity diffusion to suicidality. Treatment is illustrated from the initial level of careful diagnosis to the first stages of the interaction to the further steps and development of the interpersonal work of the dyad patient-therapist, including powerful enactments. In accessible language that references psychodynamic and relational psychoanalytic theory, the book proposes a revision of the etiopathogenesis of personality disorders, starting from the traumatic interpersonal exchanges (early relational trauma, maltreatment, deprivation, and abuse). The book breaks new ground on several levels. For the first time the body is accorded full attention in the treatment: developmentally and epigenetically situation as it is "in-between" the self and the other (at first, the caregiver, then in other circumstances of upbringing and traumatic personal relationships). The body is viewed as the main vehicle of this dysfunctional development, so that both the body and the subject are at once the "victim"—the recipient of the dysregulation resulting in impulsivity, destructiveness, self-harm, or eating disorders—and the internalized persecutor, i.e. the abuser of one's own body that sometimes also becomes the aggressor of others. Profoundly humane and scientifically sound, this book is a must-read for professionals, clients, and families involved in the difficult task of relieving the symptoms and reorganizing the personalities of subjects living in "borderline bodies."
